Why this page does not rank them
Nearly every published answer to this question is an ordered list, and an ordered list requires a weighting: a decision about which failure matters most. That decision belongs to the organization carrying the risk. Publishing an order does not remove the weighting, it hides it inside a position number that looks like a measurement.
Two health systems can correctly reach opposite conclusions about the same scribe. One deploying into a specialty where a documentation error is caught downstream by an existing review step will weight autonomy and oversight lightly. One putting the output straight into a record that drives coding and billing will weight it above everything else. There is no order that is right for both.

Read the top of that table and the category looks mature. Read the bottom and a different picture appears: the axes where nobody clears the bar are the ones about accountability rather than capability. Ambient documentation is a solved product problem and an unsolved disclosure problem.
