Comprehend Health

Comprehend Health, also sold as Comprehend PT, is an AI scribe built for physical therapy. It listens to a patient visit, or to a short summary the therapist records afterward, and writes the daily note, evaluation or progress note in the therapist's own style. A browser extension then finishes the note in the clinic's web based EMR with one click, with support for WebPT, Raintree, Jane, Prompt, Net Health, Clinicient, Stride, Empower, Fusion, PT Everywhere and many others, so therapists do not copy and paste.

Its customers include some of the largest physical therapy groups in the country. Confluent Health rolled it out to more than 1,500 clinicians and reports more than ten hours saved per clinician each week and a 3.6 times drop in staff turnover, and Florida Orthopaedic Institute onboarded 230 providers in ten weeks. The company was started by a physical therapist and a software engineer.

Pricing is open: $99 a month per provider, or $949 a year, with a seven day free trial and a promise of a refund if a clinic is not satisfied within 30 days, and enterprise terms for large groups. Every user signs a business associate agreement, audio and transcripts are deleted once the note is generated, and the company reports SOC 2 Type II with continuous monitoring through a Vanta trust center.

Capability Axes

An AI Health Index grade measures what a buyer can verify from public sources on the date shown. It is not a rating of how good the product is. A vendor can build an excellent system and grade low on an axis because it publishes nothing an outsider can check. How grades read

AI Capability
AA on AI CentralityThe artificial intelligence is the product. Remove the model and there is nothing left to sell.
Vendor Published

Comprehend exists to write therapy notes. It transcribes a physical therapy visit, or a short summary the therapist records afterward, and writes the note in the therapist's own style, covering daily notes, evaluations and progress notes, and a browser extension places the finished note in the clinic's EMR.

The extension, the EMR Playground where clinics try it against their own system and the onboarding team all exist to carry the model's output into a therapist's day. Strip out the models and nothing is left to sell, which is how Comprehend prices itself: per provider, against hours of documentation it takes off each therapist's week.

BB on Autonomy and Oversight ModelThe oversight structure is described and one part is missing, commonly the threshold at which the system stops or what happens after it is wrong.
Vendor Published

The therapist finishes every note. Comprehend drafts it in the therapist's style, the therapist reviews and edits it, and the extension then places it in the EMR with one click, so nothing is filed without the therapist acting. Comprehend also suggests wording therapists can use to ask patients for consent to record.

The missing piece is support for the review itself. Nothing links a statement in the note to the part of the visit it came from, and in physical therapy, where notes carry measurements, functional scores and plans of care that payers audit, the therapist has to catch any figure the model got wrong without a pointer to the source.

CC on Model and Technology TransparencyThe architecture is described in general terms with nothing identified. Proprietary is asserted rather than explained.
Vendor Published

The architecture is described in outline: a transcription provider converts the visit audio, the system drafts a note shaped by each clinician's past edits and style, and the browser extension writes it into the EMR. The founders describe the platform as built on AI work one of them began years before the company launched.

Neither the speech engine nor the language model is identified, by provider or version, and no release notes track changes. A clinic cannot tell what produces its notes or when the model underneath has been updated.

CC on Model Supply Chain DisclosureThe architecture is described and no model provider is named. Naming a hosting provider alone does not lift a record out of this band. Record the host in the note, because it matters for residency and breach scope, and grade on the model layer, which is the question this axis is named for.
Vendor Published

The chain is described by category without names. The privacy policy says Comprehend shares information with hosting, infrastructure, transcription, analytics, security, billing and customer support providers, bound by confidentiality and data protection terms and by business associate obligations where applicable.

No transcription service, language model provider or cloud host is named, and there is no public subprocessor list outside the trust center. A clinic therefore cannot see before signing which outside companies hear its patients' visits or whether any of them may retain or learn from that audio.

CC on Clinical and Operational EvidenceNamed customers, or vendor reported percentages with no method, denominator or reference standard. Scale of use is recorded here and is not treated as evidence of benefit.
Vendor Published

Large named deployments carry company reported results. Confluent Health rolled Comprehend out to more than 1,500 clinicians, and the case study reports, within 90 days, more than ten hours saved per clinician each week, notes signed seven times faster, 99 percent of notes signed within the week and a 3.6 times reduction in employee churn, drawn from note timestamps before and after go live rather than surveys. Florida Orthopaedic Institute onboarded 230 providers in ten weeks, with 103 power users by week ten and engagement per provider up 72 percent.

The Confluent figures come without dates for the before and after periods, the baseline hours or how hours saved were derived from the timestamps, so they cannot be checked. Note accuracy and compliance are not measured at all. The value calculator labels its own 7.5 hour assumption as a conservative blend of case data and published scribe research.

BB on AI Safety and PHI StewardshipCategorical commitments are published, such as no training on customer data, without the retention schedule behind them.
Vendor Published

Audio recordings and transcripts are securely deleted once they are no longer needed to generate the note, unless the clinic's agreement or record retention law requires keeping them, and clinics can ask for their data to be deleted. The browser extension uses page data only for its one purpose under Chrome's limited use rules.

Training use is not addressed: nothing says whether notes or clinician edits, which teach the system each therapist's style, are used to improve models across customers or by outside providers. Beyond clinician review, nothing explains how invented findings or measurements in a note would be caught.

Regulatory and Compliance
BB on HIPAA and BAA PostureBusiness associate status is stated and supported by a substantive privacy document, with the agreement or its scope not fully published. For a vendor outside the United States, an equivalent regime documented to this depth grades here. A vendor whose product does not process protected health information grades here too when it says so plainly and explains the scope, with a service privacy document behind it: stating a position a buyer can rely on is the posture this axis grades, and the band above is closed to it because there is no agreement to publish.
Vendor Published

Every user signs a business associate agreement, and the privacy policy is written for the service itself rather than only the website. It lists the patient data Comprehend handles, audio recordings, transcripts, generated notes, clinician edits, patient identifiers and appointment context, and describes HIPAA safeguards, encryption in transit and at rest, and role based, audited access.

Its terms are seen only after signing up. The privacy policy says hosting, transcription, analytics, security, billing and support providers are bound by business associate obligations where applicable, which leaves open which of them actually sign one.

BB on Security Certifications and Trust CenterA recognized certification is named in the vendor own material without the artifact, or with a scope or renewal question the buyer has to raise. A recognized certification here is an audit of how the vendor operates security, such as SOC 2, HITRUST or ISO/IEC 27001. A certification has a scope and a clock, and both are part of this grade. A certification the vendor attaches to its data center, hosting facility or cloud provider, rather than presenting as its own, is the host's and does not count here.
Vendor Published

SOC 2 Type II is named in Comprehend's own FAQ, which says the company meets the standard through ongoing audits and continuous monitoring with Vanta, and a Vanta trust center is linked for details of controls and certifications. That wording leaves open whether a Type II report has been issued or an audit is still under way. No report date, auditor or scope is given in writing, and the trust center opens only as an interactive app.

The privacy policy describes encryption in transit and at rest, role based and audited access, and regular testing and monitoring. No HITRUST or ISO 27001 certification is claimed.

CC on FDA and Regulatory StatusNo device claim is made and the product is scoped accordingly. Most administrative and operational products sit here and are not penalized for it, because this axis grades the appropriateness of the positioning rather than possession of a clearance.
Vendor Published

No regulatory position is stated, and none is needed for what the product does. Comprehend documents the therapist's own visit, turning what was said and done into a note the therapist reviews and signs, and it is not marketed as recommending treatment, diagnosing or setting a plan of care on its own.

The regime that matters to its buyers is payer documentation rules, which Comprehend addresses in its value case through audit risk and complete notes rather than through any regulatory claim. No intended purpose statement sets that scope in writing.

DD on AI Governance and Bias DisclosureNothing published on how model behavior is governed or tested. Multilingual operation with no subgroup performance sits here when the vendor markets recognition quality as a strength, because a caller the system failed to understand leaves no complaint and no record.
Vendor Published

Nothing describes how the scribe's accuracy is tested or governed. Comprehend tracks adoption closely after go live, with power user rates, engagement growth by week and counts of users at risk of dropping off, but offers no evaluation of what its notes get right or wrong.

Physical therapy visits involve exercise instruction, movement, background noise in open gyms and patients of every age and language, and no results are given by setting, visit type, accent or language. A clinic cannot tell whether notes are less reliable for some therapists or patients than for others.

CC on AI Liability and RecourseMechanisms exist that let someone challenge an output, such as audit trails, source traceability or review before commit, with nothing standing behind the output and no route for the harmed party.
Vendor Published

The therapist's review and signature are the check on each note, and access and changes are audited. Comprehend offers a 30 day money back promise, so a clinic that does not see the value it expected can recover its first month.

That promise covers satisfaction with the product, not what a note says. No error rate, warranty on note accuracy or remediation commitment for a note that fails a payer audit is offered, even though the value case leans on audit losses that complete notes prevent. The terms of service, which set the limits of Comprehend's liability, are a separate document from the product's own promises.

Integration and Deployment
BB on EHR and Interoperability DepthNamed systems with read access or one directional writing, or standards support with named deployments behind it.
Vendor Published

Notes reach the record through a browser extension that works with nearly any web based EMR, and Comprehend names WebPT, Raintree, Jane, Prompt, Net Health, Clinicient, Stride, Empower, Fusion and PT Everywhere among them, with a longer list on its demo form. The extension fills the note in the EMR's own page with one click, which is one directional writing into named systems, and large groups such as Confluent Health run it in production.

Nothing comes back from the EMR into Comprehend, such as the plan of care or prior visit measurements, and desktop EMRs that do not run in a browser are outside the extension's reach. No API or marketplace integration is described.

DD on Deployment Model and Data ResidencyNothing published about where the system runs or where the data rests.
Vendor Published

Where Comprehend runs, and where recordings, transcripts and notes are stored, goes unstated. The privacy policy refers to hosting and infrastructure providers by category, without naming a cloud provider, region or tenancy model. The browser extension works inside the clinic's own EMR session in Chrome, so each finished note also passes through the therapist's browser on its way into the record.

The company's Vanta trust center is the place such details would sit, and it opens only as an interactive app rather than a set of statements. Until the location of patient data is stated in writing, a clinic has to ask whether processing by the transcription and model providers stays in the United States.

Commercial
AA on Commercial TransparencyPublished tiers with figures, a stated unit of charge, and a route to start without a sales conversation.
Vendor Published

The price is listed and a clinic can start without a sales call. The Chrome extension plan costs $99 a month per provider or $949 billed yearly, with a seven day free trial and no credit card, and Comprehend promises a refund if a clinic is not satisfied after 30 days. Enterprise terms for large groups are quoted.

A value calculator on the pricing page lets a clinic enter its provider count, waitlist, turnover and audit concerns and see estimated monthly value against the subscription, with every assumption cited, from net revenue per physical therapy visit to the cost of replacing a provider.

AA on Setting and Specialty CoverageWhere the product is validated to operate is named and supported, settings and specialties both, whether the coverage is broad or deliberately narrow.
Vendor Published

Outpatient physical therapy is the deliberate scope, and it is backed by some of the largest groups in the field. Confluent Health, with more than 1,500 clinicians, and Florida Orthopaedic Institute, with 230 providers, run it at scale, and smaller clinics such as Active Care Erie, Buffalo Performance Physio and Modern Manual Therapy are quoted by their own therapists.

The product's EMR list, from WebPT, Raintree and Net Health to Jane and Prompt, matches the systems physical therapy clinics actually use. Occupational and speech therapy, inpatient rehabilitation and settings outside outpatient therapy are not part of the stated market.
