Health Universe

Health Universe brings patient records and AI agents into a shared clinical workspace. Its own Navigator system answers questions about a record, prepares oncology summaries and supports clinical trial workflows. Care teams can inspect the material behind an answer and ask follow up questions, while scheduled or event triggered workflows prepare work before a visit.

The platform combines record retrieval through national exchange networks and FHIR with tools for configuring, evaluating and controlling agents. Customers can use Health Universe's agents, deploy their own applications or connect other suppliers. New York Cancer & Blood Specialists uses its oncology record workflow, and the company develops clinical trial tools with Duke Clinical Research Institute. Based in San Francisco, Health Universe sells to health systems, specialty networks, academic centers and life sciences organizations. Its platform also supports governance and ONC certification documentation for decision support applications.

Capability Axes

An AI Health Index grade measures what a buyer can verify from public sources on the date shown. It is not a rating of how good the product is. A vendor can build an excellent system and grade low on an axis because it publishes nothing an outsider can check. How grades read

AI Capability
BB on AI CentralityThe model is the engine of a core module. The platform carries other value, but this capability does not exist without it.
Vendor Published

Health Universe supplies its own clinical reasoning, oncology extraction and trial workflow agents through Navigator. Those functions depend on learned models, while the surrounding platform also provides record retrieval, application hosting, access controls and certification support that have value beyond any individual agent.

BB on Autonomy and Oversight ModelThe oversight structure is described and one part is missing, commonly the threshold at which the system stops or what happens after it is wrong.
Vendor Published

Clinicians can question Navigator's responses and inspect the supporting sources and confidence signals. Organizations can withdraw access to an unsafe application, and governance committees can review logs before deployment. Exact clinical escalation thresholds and the conditions requiring a person to approve each action are not specified.

BB on Model and Technology TransparencyThe approach or the suppliers are named without the version and update discipline behind them. Naming a supplier is the entry to this band both here and on Model Supply Chain Disclosure, which ask different questions of the same disclosure: who receives the data, and what produces the output.
Vendor Published

The oncology extraction pipeline uses GPT 4o, multiple reasoning steps and structured schemas tailored to cancer type. It combines findings across documents into a patient profile. Model release identifiers, training data composition and the update schedule for the deployed agents are not specified.

BB on Model Supply Chain DisclosureSubstantial partial disclosure, or a chain that is structurally short: an in house build, a cleared model that cannot be quietly swapped, or a deployment where the transfer does not occur at all. Naming the model provider exits the band below into this one; the axis rises from here on the completeness of the party list and on the terms that govern data once it arrives.
Vendor Published

GPT 4o identifies OpenAI's model family within the oncology pipeline, and Kno2 supplies the connection to the national record exchange. A complete inventory of model processors and infrastructure partners, with each recipient's retention and training terms, is not available for the full platform.

BB on Clinical and Operational EvidenceNamed deployments with dated outcome figures and enough method to test them, or published research short of independent validation.
Vendor Published

An internal March 2026 Navigator evaluation reports 73.3% average accuracy across nine MedHELM tasks. The table includes scoring methods and sample counts for eight tasks, with mixed results against the comparison model. This benchmark evaluates the reasoning layer alone and does not establish effects on patient outcomes.

BB on AI Safety and PHI StewardshipCategorical commitments are published, such as no training on customer data, without the retention schedule behind them.
Vendor Published

The privacy terms permit training and commercialization using deidentified submitted data, while excluding records retrieved through TEFCA from those uses. Routine retention windows for identifiable records are unspecified. The safety framework describes constrained prompts, retrieval, monitoring and application removal, but no complete incident response protocol with clinical escalation thresholds.

Regulatory and Compliance
AA on HIPAA and BAA PostureBusiness associate status is stated, the agreement is available to read before contracting, the tier it applies at is clear, and the agreement discloses that it binds subcontractors who handle protected health information. Which parties those subcontractors are is graded on Model Supply Chain Disclosure and is not counted again here. Where any of the four cannot be verified from published material, the grade is the band below.
Vendor Published

Health Universe acts as a business associate for submitted PHI. Its standard business associate terms apply when a negotiated agreement is absent, including to authorized individual clinicians acting for a covered entity. The terms bind PHI handling subcontractors, require breach reporting and address return or destruction after termination.

BB on Security Certifications and Trust CenterA recognized certification is named in the vendor own material without the artifact, or with a scope or renewal question the buyer has to raise. A recognized certification here is an audit of how the vendor operates security, such as SOC 2, HITRUST or ISO/IEC 27001. A certification has a scope and a clock, and both are part of this grade. A certification the vendor attaches to its data center, hosting facility or cloud provider, rather than presenting as its own, is the host's and does not count here.
Vendor Published

The security program includes SOC 2 Type II coverage and an annual audit of security, availability and privacy controls. Health Universe also describes annual independent penetration tests, encryption, access controls and immutable activity logs. The audit report, current examination period and detailed scope are not available without further inquiry.

CC on FDA and Regulatory StatusNo device claim is made and the product is scoped accordingly. Most administrative and operational products sit here and are not penalized for it, because this axis grades the appropriateness of the positioning rather than possession of a clearance.
Vendor Published

The platform supports record review, summarization and clinical workflows, with diagnosis and treatment responsibility assigned to clinicians. Its DSI platform version 1.0 has a named ONC health IT certification dated November 13, 2024. That certification covers decision support infrastructure and does not establish FDA device clearance for its agents.

AA on AI Governance and Bias DisclosureA bias or fairness evaluation with a stated method, subgroup performance, or an independent audit of model behavior. Subgroup performance means published results compared across the groups the product serves, clinical and physiological as well as demographic, so a buyer can see where it underperforms.
Vendor Published

The oncology evaluation compares extraction performance across six cancer types using expert curated reference data. F1 scores range from 90.7% for colorectal documents to 95.1% for breast documents. These results expose variation across clinical groups; they do not describe demographic performance or establish the same behavior across every hosted agent.

BB on AI Liability and RecourseA published falsifiable commitment, or a real correction route the affected person can exercise against the vendor. A published error rate with its method and denominator grades here. A statutory right that runs to the covered entity rather than to the vendor does not reach this band on its own: every other route here asks something of the vendor, and being located in a particular jurisdiction is not conduct.
Vendor Published

Navigator's benchmark supplies task definitions, scoring methods and sample counts alongside accuracy results, making those claims testable. Clinicians can trace an answer and challenge it through dialogue. The standard terms disclaim output warranties and leave clinical decisions with the customer; no general compensation promise attaches to an incorrect answer.

Integration and Deployment
BB on EHR and Interoperability DepthNamed systems with read access or one directional writing, or standards support with named deployments behind it.
Vendor Published

The record service retrieves information through TEFCA via Kno2 and supports SMART on FHIR access within clinical workflows. New York Cancer & Blood Specialists is a named deployment using the record and summary workflow. A documented production connection that both reads and writes a named major EHR is not specified.

CC on Deployment Model and Data ResidencyA single hosted option with location implied rather than committed.
Vendor Published

The platform runs in a hosted environment with isolated organizational workspaces and permissions for people, records and agents. Organizations can bring their own applications into that environment. A committed hosting region, a complete processing geography and a customer controlled private deployment option are not specified.

Commercial
CC on Commercial TransparencyNo price is published and the posture is discoverable: a buyer can establish how the product is sold and what drives the cost before contacting the vendor. Most of the index sits here.
Vendor Published

Sales begin with a demo and pricing conversation. The DSI subscription can be charged by provider, site or EHR, with possible hosting, compute and integration fees. Current enterprise prices and the charging basis for Navigator's own agents are not specified, so the infrastructure units do not establish a complete agent budget.

BB on Setting and Specialty CoverageCoverage is named with validation behind part of it.
Vendor Published

The workflows target specialty physician networks, laboratories, senior care organizations, navigation teams and academic researchers. Oncology has named production use at New York Cancer & Blood Specialists and an extraction evaluation spanning six cancer types. Comparable validation across all the other settings and agent workflows is not specified.
